Criteria 1. Select the “Letter to Minister” icon, fill in your details and send the letter provided to the Health Minister registering your support. Please note that in order for your application to be processed, you must complete the attached letter.

Criteria 2. If you have worked in Western Australia as a nurse and registered in a category with the Australian Health Practitioner Regulation Agency or the Western Australian Nurses Board for a minium of ten years (accumlatively). Complete the attached application form and post to Medals for Nurses.
